The PHSO is hiring for a Community Care Manager position to support our Health Care Partner, Alice Hyde Medical Center!

This position is hybrid, requiring an on-site presence at Alice Hyde Medical Center in Malone, NY, while also permitting remote work from home.

WHO WE ARE:

The UVMHN (University of Vermont Health Network) PHSO (Population Health Services Organization) is a physician and payer partnered operation that provides the right constellation of services (e.g. care management and care coordination, data and analytics, provider reporting, Quality, Risk Adjustment, and Value-based Contract Performance, etc.), that enables the health network to successfully perform in High Value Care and Value-Based Contracts.

JOB DESCRIPTION:

The incumbent coordinates and delivers an innovative approach to specialized identified patients eligible for the Multi-Visit Patient (MVP) Program. They promote effective utilization and monitoring of health care resources; and assume a critical role beside and in alignment with the interdisciplinary teams to achieve optimal patient-centered, clinical, and resource outcomes. They are responsible for managing the MVP Population through the MVP program and process in alignment with the programmatic policies and procedures. Actively participates in mentorship, training, and process improvement within their assigned organization and throughout UVMHN.

EDUCATION:

Master's in social work from an accredited school of social work. LICSW preferred.

Or

Current RN licensure or compact licensure recognized by the State of Vermont required. The University of Vermont Health Network is moving toward an all Bachelor of Science in Nursing (BSN) workforce. Current internal RNs are strongly encouraged to pursue a BSN to support this initiative.

Case management accreditation required by a nationally recognized accrediting body for case management (examples: CCM, ACM or ANCC certifications), preferred.

EXPERIENCE:

2-3 years of clinical experience in a healthcare setting (or equivalent)

Care Management/Care Coordination experience required
