Job Summary:
The Hybrid Care Coordinator (CC) is a critical member of the clinical care team. He/she is office-based and responsible for providing administrative support to the clinical team as they provide cost-effective, high-quality health care for patients. Responsible to the clinical team and reports to the Care Coordination Supervisor.
- Provides in-office support 3 days a week at a minimum- this includes but not limited to: Outbound mailings, sorting and scanning incoming mail as appropriate.
- Compile, process, and maintain external medical record requests.
- Provide administrative support to the clinical care team.
- Answers incoming phone calls for general inquiries and redirect as appropriate.
- Route incoming faxes and assign as appropriate.
- Assist with the implementation or finalization of patient orders that do not require an RN to complete.
- Facilitate medical records management for outside documents that are critical for patient management, i.e. laboratory results, discharge summaries, medical documentation from specialists and primary care provider.
- Support scheduling appointments and reminder calls or texts.
- Assist with referrals and referral management
- Assist with follow-up to ensure needed DME is ordered and delivered.
- Support a variety of data entry functions.
- Provide oversight for all Care Coordination functions including but not limited to orders, referrals, disenrollment, scheduling, error processing, medical records and consent uploading, mail retrieval and sorting, email processing, etc.
- Complete other duties that support the clinical care team as assigned
