As an Aging Care Manager 2 for the Assessment Unit, you will be assigned approximately 35 to 40 cases each month through the AAA intake and referral process. You will be responsible for conducting the initial field visit with consumers, their family members, and others, either in their homes or long-term care facilities, in order to determine an appropriate level of care. You must exercise strong critical thinking skills and the ability to accurately and efficiently prioritize work based upon assigned cases and the safety and needs of the client.
