A Brief Overview
Responsible for coordinating all aspects of care for patients and families for the Center for Lymphatic Imaging and Intervention - a premier and unique Frontier Program at CHOP. Scope of responsibilities include facilitating care from patient's initial referral through the patient care experience, treatment and follow-up. Maintains relationships with referring physicians to assure appropriate contact, patient follow-up and maintenance of referral patterns. Provides patient and family education and on-going patient and family support. Ensures timely referral to appropriate support services, sub-specialists and educational opportunities throughout the CHOP network and regionally. Possesses knowledge of cardiovascular single ventricle physiology, lymphatic flow disorders, as well interventional and surgical interventions and treatment options for these highly complex conditions.
Practices as a proficient to expert practitioner of nursing, with specific knowledge of Lymphatic related disorders. Demonstrates expertise in caring for patients with multiple and complex medical problems complying with administrative policies and clinical protocols as outlined in policy and procedure manual. In addition to demonstrating clinical knowledge and leadership, may supervise identified staff. Serves as a resource and consultant to staff and participates in endeavors to increase nursing knowledge as related to Lymphatic disorders and as well as cardiovascular diseases. Demonstrates clinical leadership skills through contribution to standards of care and division goals. Contributes to clinical research initiatives as needed.

What you will do

  • If in a supervisory role:
  • Supervises the evolution of the preparation process for weekly Lymphatic clinical review conferences which facilitates patient selection criteria; this is very unique to a dynamic frontier program in which clinicians are learning from breakthroughs found every day and is currently the only one of its kind in the world.
  • Incorporates professional standards of practice and meet regulatory requirements into nursing care provided.
  • Assures appropriate processes are in place assigned to meet patient care/program needs, and reports gaps to the program manager.
  • Effectively manages gaps in process or communication for referral to treatment workflow.
  • Develops creative coverage strategies with peers and nurse navigators when necessary for supporting the work of the team.
  • Predicts challenges and obstacles and plans for how to minimize a negative impact on the patient/ family and colleagues
  • Investigates patient/family complaints and collaborates with the Program Manager, and when needed HR Business Partners and Clinicians to provide prompt feedback.
  • Partners with nursing staff to conduct educational programs to meet the ongoing professional needs of staff; works with members of the staff to coordinate the orientation of new staff.
  • Supports educational needs of team and internal/external customers as needed in order to provide patient and family education.
  • Practice:
  • Practice: Follows Nursing Process to deliver safe care: assessment, planning, implementation, and evaluation
  • Organizes the weekly clinical conference patient list for the Lymphatic Center multidisciplinary team; documents the care plan in electronic health record and facilitates the outcome through collaboration with multidisciplinary team.
  • Collaborates with other health care team members, referring physicians and families in planning and coordinating follow-up care prior to discharge.
  • Provides nursing support as needed for outpatient clinic visits, inpatient consult service
  • Synthesizes and evaluates complex assessment data to best determine priority of care needs; pertinent data is shared with Lymphatic care team, with appropriate tasks to be delegated
  • Leads family, team members and other caregivers and coordinating all facets of patient care planning with the multidisciplinary team.
  • Manages communications for letters of medical necessity, facilitates transport insurance conversations, and drives other communication with payers and families as needed, delegating to the team's administrative assistant when clerical help is needed
  • Triages phone calls for new Lymphatic referrals and existing patients and families. Communicates with families educating them on the plan of care, and documents all necessary communications
  • Evaluates patient care outcomes, communicates changes to physicians, families and other team members and contributes to necessary system changes.
  • Documentation contains an evaluative component which contributes to changes in current hospital standards, policy and procedures.
  • Contributes to evidenced based standards, policy and procedures that meet professional standards and regulatory requirements.
  • Professionalism/Leadership:
  • Holds self and others accountable for appearance and behaviors that promote a positive image of Nursing.
  • Mentors other nurses in how to interact with parents/patients and colleagues with respect and dignity.
  • Mentors and holds other nurses accountable in how to spend the extra time with families while listening for avenues to improve on the ideal patient experience and anticipate patient/family needs.
  • Role models and mentors other nurses on providing accurate patient status reports to care team members that reflects critical thinking skills that address nursing interventions, patient outcomes and alterations to plan of care.
  • Facilitates, chairs, and/or participates in division/departmental Shared Governance council.
  • Promotes/contributes to practice at local, regional and national levels.
  • Presents at regional and national conferences. Writes for publication.
  • Facilitates and promotes change in the division/ practice environment.
  • Leads division/department improvements and measures outcomes.
  • Research:
  • Reads and analyzes nursing and other scholarly articles and research studies, and formally disseminates information; applies research findings to practice and select departmental committees.
  • Initiates opportunities to participate in clinical research studies and answers complex research questions.
  • Learning
  • Applies education and training from departmental in-services and external sources to unit and select departmental practices to enhance Nursing practice.
  • Develops and utilizes patient education materials relevant to patient population and area of expertise.
  • Demonstrates health care informatics competency in order to manage patient care, outcome data and current and emerging health care technology.
  • Resource Management:
  • Ensures staff compliance to systems that promote quality, efficient and cost effective care.
  • Monitors unit usage of supplies and equipment, implements and evaluates systems for improvement to minimize waste and/or misuse of patient supplies.
  • Conducts small projects that address resource management solutions.

Education Qualifications

  • Bachelor's Degree Nursing Required

Experience Qualifications

  • At least two (2) years Pediatric experience. Required and
  • At least one (1) year Supervisory or leadership experience. Preferred
  • Pediatric cardiovascular nursing inpatient experience Preferred and
  • Neonatal nursing experience Preferred and
  • Experience educating patients and families about conditions and treatments Preferred

Skills and Abilities

  • Computer Competency:
  • Basic use of a computer (mouse, keyboard, printer, USB ports).
  • Basic use of Microsoft Office (Word processing, spreadsheets, presentations and Skype).
  • Web browsing, intranet search, document access.
  • Use of shared network file space.
  • Basic use of patient facing technology. (e.g. IV pumps, point of care devices, CR monitors).

  • Navigation of the electronic health record system.
  • Ability to locate and review specific patient data in various clinical information systems.
  • Effectively uses clinical decision support tools.
  • Understands policies and procedures, confidentiality, security, and privacy regulations regarding the use of clinical information systems.
  • Ability to collect clinical data using quality improvement tools.
  • Understanding of single ventricle physiology (Preferred proficiency)

Licenses and Certifications

  • Registered Nurse (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required or
  • Registered Nurse (New Jersey) - New Jersey State Licensing Board - upon hire - Required and
  • Basic Life Support (BLS) - issued through on organization that requires a hands-on instructor lead psychomotor skills verification (ex. American Heart Association or Red Cross). - upon hire - Required
