Job Description/Essential Functions:
Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substances abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations, Horizon House provides services to a diverse population focusing on individual strength and choice.

The Nurse Practitioner is a key member of the Case Management services provided in our Behavioral Health Services Department. This position provides psychiatric assessment, evaluation and diagnosis of individuals. Attending and participating in participant case conferences, overseeing counselors and managers in the formulation and monitoring of treatment plans are also functions of the Nurse Practitioner. Clinical supervision and psychiatric treatment of all participants by monitoring and ensuring the provision of an acceptable level of clinical practice, in the compliance with funding source requirements and agency policies and procedures are critical components of this position. Some other duties include:
  • Provide direct support and clinical intervention and/or identify key individuals and resources that can provide direct and immediate support during a crisis situation as necessary.
  • Review and provide authorizing signature on participants' rehabilitation plans and interim reports.
  • Provide consultation, conduct evaluations, and reviews.
  • Participate in planning and development sessions.
  • Maintain knowledge of current trends and developments through review of literature and periodicals, attendance at seminars and through contacts with other specialists.
Job Qualifications:

A successful applicant will have the following:

  • Certified Psychiatric Nurse Practitioner license in the state of PA.
  • Prior experience in servicing community mental health facilities and their psychiatric needs.
  • Comprehensive knowledge of psychiatric clinical and psychosocial rehabilitation practices.
  • Experience with electronic health record systems.
  • Excellent communication skills, both oral and written.
  • Strong interpersonal skills.
  • Very strong organization and analytical skills.
  • Detail oriented.
  • Valid driver's license with an acceptable driving record.

SCOPE:

  • Frequent talking and listening.
  • May require verbal crisis intervention assistance in emergency situations.
