NS II is a Residential Drug and Alcohol program that caters to homeless adult men who have a Homeless Code (111 code) with CBH. The compensation for this position ranges from $22.81 to $25.07 per hour based on credentials and experience.

Position Summary

The Therapist functions as a member of a progressive, interdisciplinary team. They impact the health of individuals, families and the community through health promotion, disease prevention and chronic disease care including monitoring and evaluating health services and outcomes and documenting services. The Therapist supports patients, families and the community to achieve their highest level of wellness, learn to live with or recover from illness or addiction, and to enhance their care capabilities and to support their right to make choices. A valued member of the treatment team, the Therapist is strategically placed to optimize exercising his/her knowledge and expertise as a counselor and educator for patients and families.

The Therapist will provide direct service care via individual and group therapy to our treatment participants. They will complete required documentation, work with family/supports, coordinate care, follow-up services and safety planning; and work with a multidisciplinary team to provide a holistic approach to care.

Essential Duties and Functions

* Provide individual and Group Counseling

* Responsible for Clinical chart documentation including: Treatment Plans, DAP notes, Case Consultations, ASAMs, and Psychosocial Assessments

* Attend and Present Clinical Case Consultations, Treatment Plan Update meetings and clinical supervisions as scheduled.

* Provide crisis intervention

* Assist in client aftercare planning and follow-up

* Facilitate specialty groups as assigned

* Evaluate client's progress and report to funders/referral sources as requested

* Determine resources needed to meet the individuals needs identified in the assessment process and coordinate community support activities

* To take all necessary actions in assuring the implementation of comprehensive service plan( treatment plan) including serving as an advocate for the client to enable him to receive needed services: identification of program needs, service gaps, and recommending strategies.

Knowledge, Skills, and Abilities

* Knowledge of behavioral health care and the social determinants of health

* Knowledge of concepts of health promotion, disease prevention, behavior change counseling, program planning, individual and group counseling

* Excellent organizational skills to manage numerous practice activities

* Proficiency in computer skills, familiarity with Electronic Medical Records and willingness to learn all computer systems

* Unconditional ability to maintain HIPAA regulations

* Leadership skills that include the ability and desire to champion teambuilding within the program

* Demonstrates effective time management skills and follow through

* Support the Center's commitment to the creation of a trauma-informed system of care.

* Demonstrate cultural competence/proficiency in interactions with others by treating co-workers, colleagues and those receiving service with respect and fairness at all times.

Qualifications

* Bachelor's Degree required, Masters preferred: From an accredited college with a major in psychology, social work, counseling, chemical dependency or similar

* At least 1+ years' experience in the drug and alcohol field required

* Current PA State certification or license strongly preferred. If not current, candidate must be willing to obtain certification or licensure following hire

* Ability to demonstrate strong written and verbal communication

* Scheduled to work every weekend based on program and patient needs

Program Summery

New Start II (NS II) RHD's New Start II at Jackson Street is a long-term residential treatment facility that provides intensive drug and alcohol treatment and mental health support services to men who are experiencing chronic homelessness. The program treatment design is an individualized level system, assisting residents in establishing psychosocial supports necessary for independent living. New Start II houses up to 16 individuals, and offers an intimate treatment setting that provides each participant unique freedom for individual programming and the opportunity to develop personal interests. New Start II staff members collaborate to provide a variety of on-site treatment services. Our staff members are eager to assist and will refer for additional supports when necessary'

Services provided include:

* Withdrawal Management & Nursing Services

* Individual and Group Therapy

* Individualized Treatment Planning

* Psychiatric Services

* Assistance with Accessing DPW, SSI Benefits, Obtaining Birth Records and SS cards

* Education and Medication Monitoring

* Referrals for Case Management Services and Housing


COVID Requirement:

RHD employees working in programs within the City of Philadelphia are required to provide proof of Covid vaccination. If not fully vaccinated, they must receive at least one shot in a two -dose series or a single dose in a one-dose series before beginning in-person shifts. Final doses must be received within 8 weeks of hire.
