Job Description

PRIMARY RESPONSIBILITY: Responsible to provide education and support to physicians/staff related to transformation efforts in the physician practice. Including but not limited to redesign of practice flow, practice workflows, implementation of standard operating procedures that deliver value and Electronic Health Record (EHR) workflow/optimization.

ESSENTIAL FUNCTIONS:

  1. Meets all standards of the Patient Satisfaction Initiative Criteria in order to make TJUH the hospital of choice for patients and employees.
  2. Provides the practice/team the facilitation and coaching of practice redesign and/or other quality and health outcome improvements with designated practices
  3. Educates physicians and/or staff on effective utilization of Electronic Health Record and/or other Population Health software programs, via one-on-one/group settings/webinars modifying teaching methods to account for the various learning needs of those being educated.
  4. Identifies opportunities for improvement of processes/flow of electronic information in the offices for optimization of documentation assisting in the redesign and rollout of best practices to all offices.
  1. Provides outreach and serves as liaison to provider organizations and physician practices for assigned program(s); builds and maintains excellent relationships with provider organizations
  2. Works with Population Health staff to create and update web-based communications and information

Bachelor#39;s degree in health or related field preferred, High school diploma or equivalent required.

EXPERIENCE REQUIREMENTS:
  1. Experience: 3 years of primary care practice office experience required
  2. Strong knowledge of electronic information systems.

Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI finger print based federal criminal records check. The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
