Horizon House, Inc. is a

developmental disabilities, behavioral health issues, and substance abuse.
Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since
1952, we have been dedicated to helping people with mental illness, drug and
alcohol addictions, developmental/intellectual disabilities, and homelessness
to stabilize their lives and to help them live as independently as possible in
the community. With over 100 locations throughout South and Northeastern
Pennsylvania and the state of Delaware, Horizon House provides services to a
diverse population focusing on individual strength and choice. We have an opening for a Residential Living Specialist (RLS) position at our Supported Living program in Bensalem, PA.
The Residential Living Specialist provides care and supervision of all assigned residents to ensure a safe, healthy, secure and pleasant environment, which promotes and reinforces resident growth and development. The RLS assists in the design and implementation of the Individual Recovery Plans and document participant progress as they move through the service delivery system. The RLS advocates for resident growth toward independence in self-care, daily living skills, socialization and community living. Some of the responsibilities include the following:
* Provide all assigned participants' basic needs and assist with skill development in the following areas: social supports, food management, self-care preservation/personal hygiene, relapse prevention, medication management, stress management techniques, money management, utilization of public transportation and participation in meaningful and planned community activities.
* Ensure participants are transported to assigned programs, appointments and/or destinations. May arrange for transportation and/or accompany participants to appointments.
* Administer medications consistent with participants' self- medication plans, in accordance with State and Agency medication administration regulations. Inventory medications and complete all appropriate forms, as assigned. Must successfully complete the agency's medication administration training.
* Complete documentation, including progress notes, daily logs, and service plans, as required by regulations. Implement and track all participants' progress. Provide direct input and follow-up reports on individual growth and development, as required.
* Establish an on-going dialogue with other members of the treatment team, including program staff, vocational/educational instructors, consultants, members of the medical profession and family members to provide continuous supports.
* Attend staff and other meetings as they relate to the participants' and their overall growth and development.
* Plan and initiate a variety of social and recreational activities for the participants.

Qualifications:
* A.A. degree in the social sciences or health care related field and one year of relevant experience in the
MH/MR field or High school diploma and two year's experience in the mental health field; or any combination
of experience, education or training that would provide the level of knowledge, skill and ability required.
* Demonstrated crisis intervention/prevention and de-escalation skills required.
* Good communication skills, both oral & written.
* Ability to work independently and as part of a team.
* Required to possess a valid drivers' license with an acceptable driving record.
Scope:
* Requires the ability to lift, carry, fold, kneel, reach, stack, stoop and bend which may include direct
intervention with participants.
* Assist participants with moving from bed to chair, chair to shower, etc.
* Frequent talking and listening.
* May require crisis intervention in emergency situations.
* Possess manual dexterity and fine motor skills.
* May be required to report to work during emergencies including inclement weather.
