Provides services to assist patients and their families meet their biopsychosocial needs by
enabling them to access and utilize medical care and community resources. The goal is to achieve expected
functional outcomes with specific regard to the patientrsquo;s age and physical limitations. Provides medical management
coordination to enhance the efficiency of delivery of care and ensure appropriate allocation of resources. Provides
consultation and teaching to the team as needed.
This position requires providing service to the following age population(s): AD, A, O, F in a manner that
demonstrates an understanding of the functional/developmental age of the individual served.

MSW, MSS, or BSW with experience preferred. Bachelorrsquo;s degree or MS in a related field may be considered based
upon experience and knowledge of care management and discharge planning. Three to five years of clinical
experience in healthcare preferred, preferably in a rehabilitation hospital.
