Primary Care Physician - Family Medicine Faculty ndash; Jefferson FAB Center for Complex Care/Primary Care at the Navy Yard, Jefferson Health

The Department of Family amp; Community Medicine of the Sidney Kimmel Medical College at Thomas Jefferson University is currently accepting candidates for the position of outpatient Primary Care Physician at the Jefferson FAB Center for Complex Care/Primary Care at the Navy Yard, Jefferson Health. The FAB (For Adolescents and Beyond) Center serves as a primary care medical home for patients with complex childhood-onset conditions including, but not limited to, Autism, Intellectual and Developmental disability, Cerebral Palsy and Spina Bifida. We aim to provide comprehensive and compassionate care to people with disabilities. The practice is located in South Philadelphia near Methodist Hospital, just minutes from Center City Philadelphia and in the heart of the exciting Navy Yard/South Philadelphia business and residential district. Responsibilities will include:

middot; Outpatient family medicine (currently limited to ages 12+)

middot; 4 FAB sessions per week and 3 primary care sessions

middot; During FAB sessions, the provider will see 5-6 patients per session.

middot; During primary care sessions, the provider will see 10-12 patients per sessions.

middot; The provider will have 10% protected time for research/HRSA grant responsibilities and 20% clinical admin time.

middot; Establishing and maintaining a panel of patients, managing chronic health conditions, acute conditions, and preventive care.

middot; Womenrsquo;s health visits, routine GYN exams, and LARCs if desired / credentialed.

middot; At-home telephone call 5 pm ndash; 8 am in a 7-10 person provider pool on weekdays and 24 hour call every 7-10 weekends.

middot; Work as part of interprofessional team (including MA, behavioral health, social workers, population health specialists) that provides patient-centered care.

Requirements: Qualified applicants will hold a MD or DO degree and be board certified, or eligible, in Family Medicine. Must have or obtain a PA medical license/DEA. Qualified applicants must have significant clinical experience (3-5 years) or a fellowship in care of individuals with complex childhood-onset conditions. Applicants should also have experience in quality improvement, scholarly activity, community engagement, and medical student/resident education.
