Job Description/Essential Functions:
Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

We are seeking qualified candidates for the position of Health Care Coordinator. The position has responsibility for providing support and direction to program participating in the CCBH BHH+ (Community Care Behavioral Health Behavioral Health Home Plus the Value Based Purchasing) initiative. Other responsibilities include the following:


Working with management to identify participant who have extensive co-morbid physical and behavioral health conditions in support of developing comprehensive wellness plans and care coordination.

Developing and implement programs to support medication adherence and medical appointments

Planning, securing and providing case specific training and consultation to staff working directly with participants.

Working with staff and participants to support positive lifestyle changes, address lapses in clinical care and assess and manage changes in clinical status.

Participant in Wellness planning developing preventative wellness plans around weight management, nutrition, health screening, exercise and smoking cessation.

Working with staff and participants to support positive lifestyle changes, address lapses in clinical care and assess and manage changes in clinical status.

Communicating regularly with participants, caregivers, families, county representatives and local health care providers to establish priorities, coordinate care and identify problem areas and needs.
Job Qualifications:

Master's Degree in Human Services with 5 years' experience in community behavioral health at least one (1) of which is supervisory or RN with three (3) years' experience in behavioral health or integrated care.

Scope:

  • Valid Driver's license.
  • Obtain or currently is a Smoking Cessation trainer.
  • Obtain or currently trained in measuring vital signs.
