We are seeking qualified candidates for the position of Health Care Coordinator. The position has responsibility for providing support and direction to program participating in the CCBH BHH+ (Community Care Behavioral Health Behavioral Health Home Plus the Value Based Purchasing) initiative. Other responsibilities include the following: Working with management to identify participant who have extensive co-morbid physical and behavioral health conditions in support of developing comprehensive wellness plans and care coordination. Developing and implement programs to support medication adherence and medical appointments
Planning, securing and providing case specific training and consultation to staff working directly with participants.
Working with staff and participants to support positive lifestyle changes, address lapses in clinical care and assess and manage changes in clinical status.
Participant in Wellness planning developing preventative wellness plans around weight management, nutrition, health screening, exercise and smoking cessation.

Communicating regularly with participants, caregivers, families, county representatives and local health care providers to establish priorities, coordinate care and identify problem areas and needs. |
