Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

We are seeking a full-time Resilience Counselor to work in our Behavioral Health Services Division, to be the primary clinician to individuals with early episode psychosis and to provide individual CBT, group CBT and family psycho-education, as well as more traditional case management services. Additional responsibilities include the following:

  • To engage in outreach and recruitment activities.
  • Complete and maintain all chart documentation according to insurance and licensing regulations including progress notes, treatment plans, concurrent reviews within established timeframes.
  • Assure each client has input into developing an up-to-date treatment plan; modify plans to adapt to special needs and/or service delivery gaps.
  • Establish an on-going dialogue with other members of the rehabilitation/treatment team, psychiatrist, vocational/educational instructors, nurses, occupational therapists, consultants, members of the medical profession and family members, as needed, to provide continuous supports; schedule planning and linking conferences as needed.
  • Serve as an advocate for clients to assume a proactive role in their rehabilitative process.
  • May perform phone and in person screenings and provide intake/assessments for individuals referred to Mental Health/Substance Abuse Services as needed.
  • Liaison with schools, employers, and the office of vocational rehabilitation to support participants.
Job Qualifications:

A successful applicant will have the following:

  • Masters-level licensed/license eligible Mental Health Professional (Social Work, Professional Counseling, Psychology, LMFT). License preferred.
  • Certified Addiction Counselor (CAC), preferred
  • Bi-lingual preferred
  • Skills and competence to establish supportive trusting relationships with persons with early psychosis (15-30 y/o) and respect for participant rights and personal preferences in treatment are essential.
  • Experience with community/home based services preferred
  • Knowledge of mental health interventions (CT-R, CBT, MI) and supports
  • Some knowledge of support systems for substance users.
  • Good communication skills, both oral and written.
  • Ability to work independently and as part of a multi-disciplinary team.
  • Must have a valid driver's license with an acceptable driving record.

Scope:
* Frequent talking and listening.
* Requires the ability to lift, carry, fold, kneel, reach, stoop and bend which may include direct intervention with clients.
* Possess manual dexterity and fine motor skills.
* Must be available for local travel.
* May require crisis intervention in emergency situations an availability after hours by phone for clinical needs.
