Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

Our Suburban Behavioral Health Services Region in Chester, PA is seeking dedicated Residential Support Specialist to provide resource management and other supports to individuals who have been identified as having mental health disabilities or who have MH & D&A disabilities. This position instructs, counsels and provides support to those individuals in residential settings or to those seeking living arrangements, with supplemental service supports.

This position is also has the responsibility to provide assistance with social, vocational, educational, recreational and daily living skills.

Assist with participant movement, goal achievement and local community interaction.*

Ensure a continuity of services, in compliance with funding source and Agency regulations.

Assess initial client needs, based on individual strengths and available resources.

Maintain and monitor an assigned caseload of clients, through casework/resource management and referral, counseling sessions, and skill teaching/refinement.

Participate in developing goals with the client, for inclusion in the individualized service plan.

Ensure provision of preventive healthcare and necessary follow- up services for clients.

Maintain and submit required program reports and documentation, including progress notes, service plan reviews, incident reports and medical/appointment logs.

Serve as an advocate on behalf of the clients and encourage them to assume a proactive role in their rehabilitation/transition process.

Maintain a close liaison with the local community and area service providers/agencies to facilitate program cooperation and provide for constructive input and/or alternative resources to current plans.

Utilize local resource networks for referral, to enhance programming continuity.

Participate in relevant training to enhance professional growth and development, with an emphasis on counseling, case management and basic living skills.

Maintain knowledge of current trends and developments in the field through review of literature and periodicals and attendance at seminars.
Job Qualifications:
B.A./B.S. (preferred) degree in Human Services/Social Sciences, Healthcare or a related field; or anycombination of experience, education or training that would provide the level of knowledge, skill and abilityrequired.

* Very good communication skills.

* Valid Driver's License with an acceptable driving record.

* Ability to work independently and as part of a team

.* Detail oriented.

* Proficiency in various computer technologies including Electronic Health Record, Outlook, MS Word and Excel.

* Fee-for-service experience and ability to maintain daily productivity standards using an Electronic Health Record

Scope:


* Requires the ability to lift, carry, fold, kneel, stack, stoop and bend which may include direct intervention with participants.

* Possess manual dexterity and fine motor skills.

* Must be available for local travel.

* May require crisis intervention to emergency situations.

* May be required to carry agency cell phone during scheduled emergency on-call rotation.
