We are seeking a qualified Behavior Health Specialist to fill the following Full time opening. This position is based in our Swarthmore, Delaware County office. The Behavior Health Specialist is responsible to conduct initial client intake and assessment for individuals referred for treatment; to provide outreach and case management services to stabilize homeless clients with serious mental illness and substance dependence; to promote self-sufficiency and independent living for client caseload; to transfer clients to other forms of care as needed; to comply with funding source regulations and agency policies and procedures; to provide on-going review and assessment of client services and growth, within established system' and to ensure continuing of services in compliance with funding sources and agency Policy and Procedures. This position is responsible for the following duties:
