The Intellectual Disabilities Supports Coordinator position consists of identifying, assessing and service planning for individuals referred to Bradford/Sullivan Counties.
As an Intellectual Disabilities Supports Coordinator you will collect information, write case histories and make recommendations. You will participate in team reviews and follow through to achieve individual's goals. You will maintain an active caseload and keep notes, document service contacts, keep a log of daily activities and complete statistical reports. Maintain contact with individuals and their sources through phone calls and site visits. Schedule appointments and keep correspondence on the individual's behalf. Confer with supervisory personnel on matters needing clarification and to seek advice about procedures. Participate in staff training, development sessions and in-service training. Transport clients when other transportation is not available. Be available for after-hours service, as necessary. Perform other related work as required.
