DESCRIPTION OF WORK

An Aging Care Manager 2 is expected to appropriately maintain a caseload of vulnerable consumers. Responsibilities involve exercising initiative and independent judgment in discharging assignments, seeking advice and help with more unusual and difficult cases, and collaborating with other community professionals, agencies, and resources. This position obtains information from the consumer, members of the consumers' families, neighbors, and other persons necessary for the identification of social, economic, emotional, health, or physical problems. Effective communication helps to ensure that consumers receive the care and support they need. Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:30 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
  • This position may be required to be on a standby rotation for off-hours coverage.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS:

Minimum Experience and Training Requirements:

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training

  • Hiring preference for this vacancy may be given to candidates who live within Allegheny, Armstrong, Cambria, Fayette, Indiana, Somerset, Washington, and Westmoreland counties. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.
