Earn $22.06/hour

Position Details

The Housing Specialist within the ACT team is instrumental in meeting the housing requirements of individuals with co-occurring disorders. Working closely with Psychiatrists and Primary Care Physicians, the Housing Specialist aids in treatment planning by evaluating the influence of addiction on mental and physical health and aiding in decision-making processes. Responsibilities encompass advising on medication prescriptions for substance abuse disorders, evaluating physical health, and orchestrating medical, psychiatric, and substance abuse treatments. By ensuring consumers have stable housing, the Housing Specialist significantly bolsters the efficacy of the ACT program, ultimately promoting overall recovery and well-being.

  • Assists consumers with housing needs, including searches, budgeting, and applications.
  • Builds relationships with housing vendors and facilitates referrals.
  • Supports lease agreements, deposits, and budgeting with consumers and payees.
  • Acts as a liaison between individuals and landlords to resolve issues.
  • Collaborates with the team on housing goals and participates in treatment meetings.
  • Completes assessments, provides cross-trainings, and fulfills on-call duties as assigned.

Our Assertive Community Treatment (ACT) model supports individuals with serious mental illnesses who may not have experienced success or haven't found effective support within traditional outpatient programs. We offer tailored services emphasizing relationship-building and active involvement in enhancing functioning and symptom management.

Services involve weekly home visits, provided either in individuals' residences or within the community, utilizing personal vehicles for client transportation and community-based tasks.

Requirements

The ideal candidate will possess the following qualifications:

  • Bachelor's Degree in the human services field is required.
  • 1 to 3 years of experience working with individuals with severe mental illness diagnoses preferred.
  • Knowledge and understanding of recovery principles and recovery-driven service systems.
  • Valid Driver's License. Travel is necessary to offer support across Dauphin County.
