Job Description

For many people living with HIV, sticking to both their day-to-day treament plan (like taking their medication consistently) and their long-term HIV care (like attending several medical check-ups per year) can be hard. It is vital for people with HIV to take medications as prescribed to get the most out of their treatment.

The Managed Problem Solving+ (MAPS+) program helps people living with HIV who have fallen out of HIV care or who are having trouble taking their medicine. Making routines and sticking to them can be tricky, so the MAPS+ program takes a personalized, creative problem-solving approach to help people overcome barriers like caregiver responsibilities, transportation problems or life circumstances to keeping up with their HIV care.

The Community Health Worker will receive intensive training to prepare them for the position as well as continued support to help succeed. The Community Health Worker will use MAPS+ with up to 35 clinic patients tohelp them manage their HIV treatment in the context of their lives. The Community Health Worker will also conduct outreach and health education services to support clients in optimal care engagement. We expect this work to be a combination of work in-person, video conference, and/or telephone.

This position is a partnership between Albert Einstein Medical Center and the University of Pennsylvania as part of a multi-site study on the MAPS+ intervention. The Community Health Worker will receive training and support in the MAPS+ intervention from the researchers at Penn and will be task and operationally supervised by the HIV team at Einstein.

Essential Functions:

  1. Complete trainings in MAPS+ and research to guarantee appropriate skills in program requirements.
  2. Follow MAPS+ intervention manual activities: conduct in-person, virtual, and telephone call sessions to get to know patients, identify support services, and collect and input data.
  3. Work with the clinical team to support patientsrsquo; medication taking and attendance to medical appointments.
  4. Meet patients outside of normal business hours, as needed, responding to calls by the end of the next day.
  5. Participate in ongoing training and supervision and keep supervisors informed of any problems that arise.
  6. Comply with all program protocols, including those regarding confidentiality of patient information.
  7. Responsible for engaging HIV infected/affected patients in basic HIV/AIDS disease management education including U=U and adherence counseling; provide harm reduction to patients, partners, and family of patient.
  8. Enhance positive healthcare outcomes and community awareness about HIV/AIDS, impacting community health through education, linkage, referrals and direct service delivery.
  9. Address patient phone and electronic messages, responding or relaying them to appropriate contact as indicated.
  10. Identify and evaluate models that transform sporadic users of health care into regular and continual users of healthcare; and identify effective methods to support and retain clients in health care.
  11. Conduct patient outreach by phone, inpatient or ED visit, mail, and electronic messaging in order to optimally retain patients in care
  12. Collaborate with inter-disciplinary team to optimally serve patient with wrap-around services.
  13. Refer patients to internal and external resources and services as appropriate.
  14. Document service delivery in EMR, CAREWare and RedCAP
  15. Complete JHF-MAI data sheet and monthly reports.
  16. Collaborate with PDPH on care re-engagement initiatives
  17. Other duties and projects related to MAPS+, outreach, health education, and reduction as assigned.

Education/Training/Experience:

  • A high school diploma or GED
  • Lived experience with HIV or experience working with or otherwise helping people living with HIV
  • Comfort working with people living with HIV and in a medical setting, and passion about working to improve the health and quality of life of people living with HIV/AIDS
  • Basic comfort with electronic systems (email, Word) and virtual meeting platforms (e.g., Zoom, Teams)
  • Willingness to receive training on medical information about HIV through
  • Experience in community-based advocacy work (preferred)
  • Experience in community leadership, like serving as a block captain, volunteering through a religious organization, or serving as a mentor (preferred)
  • Ability to speak both English and Spanish (preferred)

Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI finger print based federal criminal records check. The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
