Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

Currently, we are seeking a Full-Time Case Manager to work in our Homeless Services Division. The position provides community-based supports to individuals in compliance with the Agency service contract and policies and procedures. Approximately 75% of work is done in the community. The selected candidate does home based visitations and interacts with members of the multidisciplinary team to ensure that participants receive individualized services designed to meet their needs and to help them successfully maintain housing and accomplish their recovery goals.

Some of the responsibilities include the following:

  • Providing engagement and case management support including home-based visitations and other services within the Philadelphia community. Approximately 75% of work is in the community.
  • Interacting with members of the multidisciplinary team to ensure that participants receive the individualized services designed to help them successfully maintain their permanent housing.
  • Providing services in compliance with contract and Agency regulations.
  • Conducting and coordinating outreach activities designed to locate and engage TCMU authorized participants.
  • Provide clinical services to participants as needed in their natural environment or in their program housing or assist participants to access mainstream clinical services.
  • Participating in the resolution of crisis situations; assist with identifying key individuals and resources that can provide direct support and/or immediate intervention during a crisis situation. In conjunction with other staff members, provides on-call crisis intervention covering nighttime and weekend hours.
  • Working with participants to develop, implement and monitor their personal goal plans to enable them to stabilize and retain housing on a permanent basis.
  • Assisting clients in the housing process, including procurement of necessary documentation, completion of housing applications, preparation for move-in, and support after move-in.
  • Serving as an advocate and encourage clients to assume a proactive role in their rehabilitation process. Provide direct support through effective communications with program staff, clients and other providers.
Job Qualifications:
  • Bachelor's degree with concentration in sociology, social work, psychology, gerontology, anthropology, criminal justice, theology, nursing, counseling or education; or a high school diploma plus a minimum of 12 college credit hours in sociology, social welfare psychology, gerontology; or other social science plus (2 years of mental health direct care experience; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required by CBH standards.
  • Must be able to communicate both verbally and in written form with every level of the behavioral health system and maintain effective professional relationships with associates in the Agency/community at large.
  • Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for participant rights and personal preference in treatment are essential.
  • Ability to work independently and as part of a focused team.
  • Working knowledge of Outlook, MSWord and Excel.
  • Ability to maintain daily productivity standards.
  • Fee-for-service and electronic health record experience strongly preferred.
  • Must possess a valid driver's license with an acceptable driving record.

Scope:

  • Frequent talking and listening.
  • Requires the ability to lift 25 pounds, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with participants.
  • Possess manual dexterity and fine motor skills.
  • Must be available for local travel and possess a valid driver's license with an acceptable driving record.
  • Must carry a mobile phone during scheduled working hours and scheduled on-call services.
  • As essential personnel, may be required to report to work during emergencies including inclement weather.
  • Requires crisis intervention and availability to respond in emergency situations.
