This Community Health Worker (CHW) role is 50% dedicated to care engagement and re-engagement of people living with HIV and 50% providing administrative support to the teleprep program. The CHW is responsible for handling sensitive, confidential medical and social information among patients, caregivers, medical providers, social workers, case managers, and navigators. Commitment to patient service, sexual health and well-being, and professionalism are paramount for success in this role. Excellent prioritization, decision-making, multi-tasking skills are also essential in this position. This role works with the clinical team to support patientsrsquo; linkage to care, medication adherence and attendance to medical appointments, participates in ongoing training and supervision, keeps supervisors informed of any problems that arise, and complies with all program protocols, including those regarding confidentiality of patient information.

For many people living with HIV, sticking to both their day-to-day treatment plan (like taking their medication consistently) and their long-term HIV care (like attending several medical checkups per year) can be hard. Yet optimal retention in care is key to ending the HIV epidemic. The Community Health Worker will work alongside the IDC outreach, viral load suppression, and other staff to engage and re-engage patients not optimally linked or retained in HIV care. They will utilize data systems to identify lost-to-care patients and work creatively to bring them back into care. They will be a part of the data-to-care (D2C) initiative that collaborates with the Philadelphia Department of Public Health (PDPH) on reducing the number of people not in care. Specific duties include but are not limited to:

  • Outreach to patients identified as not optimally retained in HIV care
  • Assess patientsrsquo; obstacles to care engagement
  • Provide direct support to address barriers to care and refer/link to support services theymay benefit from
  • Participate in outreach activities in the community and in-reach activities in thehospital
  • Document services in CAREW are database and electronic medical record
  • Participate in data-to-care activities, including updating and completing monthly spreadsheet, contributing to case conferences, and D2C meetings both internally and with PDPH

Low barrier access to HIV prevention services is also central to the IDCrsquo;s mission and work to end the HIV epidemic. The CHW will enhance patient experience by providing administrative and technical support with the teleprepprogram, including but not limited to:

  • Appointment scheduling/re-scheduling
  • Appointment registration, insurance verification, and check-out
  • Schedule management; review schedule weekly to confirm correct providers,appointment and encounter types; block sessions when needed
  • Support Program Supervisor with invoice preparation, arrange payment for unreimbursed services, and program reporting
  • Liaise with at-home testing vendor and support smooth procurement of testing kits
  • Prepare and complete various internal requisitions
  • Data entry

Education/Training/Experience:

  • A high school diploma or GED
  • Lived experience with HIV or experience working with or otherwise helping people living with HIV
  • Comfort working with people living with HIV and in a medical setting, and passion about working to improve the health and quality of life of people living with HIV/AIDS
  • Comfort and skill utilizing electronic systems (email, MS Excel, electronic chart and scheduling systems) and virtual meeting platforms (e.g., Zoom, Teams)
  • Willingness to receive training on medical information about HIV treatment and prevention
  • Experience in community-based advocacy work (preferred)
  • Ability to speak both English and Spanish (preferred)

  • Friendly personality and love of working with people from different backgrounds (various ages, sexual orientations, gender identities, races/ethnicities, and socio-economic circumstances)
  • Ability to conduct sensitive, empathetic interviews and conversations that respect the dignity and diversity of clients to build trust and effective relationship with them
  • Strong problem-solving skills that require patience, creative thinking, and a positive attitude
  • Empathy, compassion, self-motivation, and a willingness to try to understand the challenges that are unique to each personrsquo;s life experience and stigma related to HIV
  • Organizational ability required to balance a lot of different tasks
  • Ability to work independently and as part of a team
  • Strong communication skills (verbal and written)
