Job Description/Essential Functions:
  • To provide and assist team members in coordinating the behavioral and physical health care needs of program participants.
  • To complete the health assessments on participants.
  • To maintain a record of medical and behavioral health history and on-going medical and treatment records. Review all charts one time per month (approximately 25 charts per week) to ensure medical needs have been met and documentation meets applicable standards.
  • To consult with and assist team members regarding development and implementation of the health care needs and goals.
  • Negotiate the health care responsibilities of participants with provider agencies.
  • Coordinate with team members' routine medical, vision, lab, psychiatric and dental appointments. Ensure a mechanism for reporting and follow-up on results. May accompany the participant to an appointment when it is determined by the nurse in consultation with the team, and facilitate communication between PCP (primary care physician) and psychiatrist.
  • Attend team meetings regularly to review medications, emergency intervention plans, and response to treatment, and to review health care needs, participant's response to treatment and coordinate scheduling appointments.
  • Monitor participant's medication. Verify insurance
    for medication approval by making phone calls to appropriate insurance and pharmacies as directed by psychiatrist.
  • Participate in discharge planning for participants from acute or extended care facilities to provide continuity of medical substance abuse and psychiatric care. Coordinate with participant's case manager and therapist for scheduling of appointments.
  • Complete required documentation. Develop, review, and revise forms, procedures and standards of care according to accepted professional practice. Conduct quality assurance reviews of health care section of medical records.
  • Provide consultation services to community agencies upon request or as needed.
  • Provide educational training services to participants, provider agencies, and non-medical staff, including medication, diagnosis and interventions training classes.
  • Perform other duties and responsibilities as they relate to the overall enhancement of program service delivery.

Job Qualifications:
  • Requires a Board Certified Licensed RN/LPN in the State of PA.
  • Prior experience in servicing community mental health & Substance Abuse facilities and their needs.
  • Comprehensive knowledge of psychiatric clinical and psychosocial rehabilitation practices.
  • Excellent communication skills, both oral and written.
  • Strong interpersonal skills.
  • Someone who is detail and team oriented.
  • Very strong organization and analytical skills.
  • Must be available for scheduled emergency on-call service.
  • May require verbal crisis intervention assistance in emergency situations.

Scope:

  • Frequent talking and listening.
  • May require verbal crisis intervention assistance in emergency situations.
  • Must be available for local travel and possess a valid driver's license with an acceptable driving record.
  • Scheduling flexibility and possible on-call work are necessary.
  • May be required to report to work during emergencies including inclement weather.
