EDUCATIONAL/TRAINING REQUIREMENTS:

Bachelor degree in health or business administration. Masterrsquo;s degree preferred.

BSN from accredited school of nursing; MSN preferred

EXPERIENCE REQUIREMENTS:

The ability to use computer software. Prefer familiarity with electronic medical records.
Minimum five years professional nursing experience, including two years supervisory level experience.

PRIMARY FUNCTION:

The Nurse Clinical Coordinator, in partnership and collaboration with other healthcare professionals, manages specific clinical and administrative functions related to the operations of the Jefferson Bridge Program clinical practice at the 833 Chestnut Suite 301 site under the supervision of the Medical Director, and Department Administrator. The Bridge Program is a new pilot initative to offer comprehensive, low-barrier care for people with opioid use disorder who have recently interfaced with the Jefferson acute care system. The RN Clinical Coordinator is responsible for organizing daily clinical workflow, working toward best practice principles/guidelines and participation in change management processes particularly toward access and continuity, Care Management, Comprehensiveness amp; Coordination, Patient amp; Caregiver Engagement, and Population Health.

As part of the nursing team, develops and coordinates support staff performing clinical activities; serves as liaison between physicians, clinical, clerical and administrative staff and front desk.

ESSENTIAL FUNCTIONS:

Administrative

  • Oversees and implements on-going training of staff and leads in-services.
  • Selects, trains/orients, and supervises clinical department personnel.
  • Works to create the ideal patient experience; resolves complaints, implements effective solutions, and recognizes and analyzes implications on new situations
  • Monitors and responds to patient satisfaction issues.
  • Works independently as a supervisor and RN.
  • Evaluates performance and recommends disciplinary actions.
  • Resolves problems in areas and ensures compliance with regulations and standards.
  • Participates in hiring, orientation, assignment, and evaluation of clinical support staff.

Operational

  • Attends team huddles/assemblies to provide announcements, clinical operations updates, review staffing, daily assignments, and assigning preceptors.
  • Works directly with medical director to develop and implement team-based policies, procedures and work standards.
  • Leads and implements change initiatives to strengthen team-based care.
  • Organizes department workloads, establishes work standards, and evaluates workflow problems and resolutions.
  • Helps establish/implement goals, objectives, policies, procedures and systems for the assigned medical assistant needs.
  • Serves as liaison between medical staff.
  • Works with medical staff to ensure quality patient care and services are provided.

Clinical

  • Assists the teams during daily tasks such as rooming, patient flow, etc.
  • Utilizes evidence-based ambulatory care nursing interventions during clinical visits, telephone encounters, and electronic communication.
  • Oversees and audits Pre-Visit Planning (PVP).
  • Performs care coordination for patients i.e. abnormal results, triage, double checking shots/vaccines, management of in-baskets.
  • Trains and supervises clinical staff in competencies (vitals, injections, etc.); assists with identifying and implementing tools and processes to create an effective and efficient patient flow. Develops and maintains clinical support staff schedules to ensure adequate coverage for clinical sessions.
  • Oversees clinical care essential to the assessment, promotion, maintenance and restoration of patientsrsquo; health and wellness.
  • Oversees patient needs and suggests solutions to patient care problems.
  • Regularly staffs nurse visits and ensures patients are scheduled appropriately.

  • Remain proficient and knowledgeable within the EMR ndash; attend JUP update training sessions.
  • Utilize and remain current with the tasking function of the EMR.
  • Interacts with co-workers, visitors, and other staff consistent with the core values of the University.
