POSITION SUMMARY: The Case Manager assesses the client and referral including, but not limited to, demographic information for parents and children, circumstances that brought the case into care, medical information for the child, court information, and goals to address at Family School to determine appropriate services for the client family. The Case Manager determines, sets, and assists clients in accomplishing their Family Service Plan goals.

DUTIES AND RESPONSIBILITIES:

  • Set goals and provide counseling to alleviate stresses interfering with a positive parent-child relationship and track progress towards completion of goals
  • Assist parents in the use of other human services resources including but not limited to, medical, mental health, and income supplemental services
  • Assesses and provides counseling on issues of child safety and appropriate parent behaviors related to specific problems in the parent-child relationship and/or family functioning
  • Communicate concerns about child safety immediately to supervisor and Department of Human Services (DHS)
  • Prepare for and support the family's attendance at center-based program and support progress of child and parent goals per DHS Family Service Plan developed by the primary case manager from the Community Umbrella Agency (CUA) and/or DHS and develops child's Individualized Family Service Plan (IFSP)
  • Complete accurate and timely family assessment forms, summary of service, quarterly, court and closing reports
  • Coordinate services including, but not limited to, mental health, drug and alcohol, anger management, housing with the Family's DHS worker
  • Conduct monthly home visits to each family /parent assigned to caseload
  • Work one Saturday per month, as assigned
  • Perform cardiopulmonary resuscitation (CPR) and crisis intervention using agency-trained protocols
  • Perform other duties as assigned

DIRECT REPORTS: None

CONTACTS: Staff, human services agency representatives, children, families

EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS:

  • Bachelor's in social work, psychology, sociology, education, or related human services field; licensed social worker and master's preferred
  • Previous experience in direct service casework preferred
  • Non-profit human service experience highly preferred
  • Demonstrated ability to work effectively as part of a team
  • Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
  • Demonstrated strong attention to detail
  • Demonstrated strong time management and organizational skills
  • Demonstrated excellent judgment with the ability to independently solve problems and make decisions with little or no need for direct supervision
  • Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
  • Must be able to perform CPR using agency-trained protocols
  • Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills
  • Demonstrated intermediate experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint; Access, Publisher and report-writer experience preferred
  • Knowledge of Electronic Health Record (EHR) / Electronic Medical Record (EMR) preferred
  • Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record
