Job Description
SUMMARY

The Early Intervention Service Coordinator is responsible for effectively and efficiently managing a caseload and assisting each eligible family in identifying, locating, coordinating and monitoring mandated services for children birth to three years of age.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Initial screening is completed for all referrals made for Early Intervention Services to determine resources, priorities and concerns of the family and to identify the supports and services necessary to enhance the family's capacity to meet the developmental needs of the child. Tracking at-risk children who do not meet the eligibility for Early Intervention Services, but who meet the criteria for tracking services;
  • The Service Coordinator is responsible to coordinate the Multi-Disciplinary Evaluation within 45 days of initial referral and facilitating and participating in the development, implementation, review and evaluation of the Individual Family Service Plan (IFSP), and transition plan;
  • The Service Coordinator is responsible for following up on any health, safety or other concerns identified at the time of the visit. Service Coordinator documents activities of monitoring in case comments and billed to MA, if applicable;
  • The Service Coordinator is responsible for assisting the family of an infant or toddler with a disability in gaining access to the early intervention services and other services, in the least restrictive environment, as identified on the Individual Family Service Plan, including assisting the family in arranging to receive medical and health services and providing the family with appropriate community resources;
  • Attends at least 24 hours of training annually, in addition to any pre-service training relevant to early intervention services, child development, community resources or services for children with disabilities.
  • Other duties as delegated.
QUALIFICATION REQUIREMENTS

  • Bachelor's degree which includes 12 college credits in early intervention, early childhood, special education, early childhood education, child development, special education, family counseling, family studies, social welfare, psychology or other comparable social sciences and;
  • One year of full-time or full-time equivalent experience working with or providing counseling to children, families or individuals with disabilities;
  • This position is subject to mandates requiring Child Abuse, Criminal History, and FBI Clearances for employment;
  • Pennsylvania residency is not a requirement for this position.

Promotions are based on meritorious service and seniority. Meritorious service is defined as the absence of any discipline during the 12 months preceding the closing date of this posting, and the last regular or introductory evaluation is higher than fair. Seniority is defined as a minimum of 6 months of service in the next lower class as of the closing date of the posting.
