Job Description
THE POSITION

Would you like a rewarding career that can make a positive difference in the lives of others? If you have a passion for helping others, then the Lycoming CYS Program is interested in talking to you!

DESCRIPTION OF WORK

This professional social service position is assigned to the Ongoing Services Unit of Lycoming Children and Youth Services. This position is responsible for the provision of the full range of case management, treatment and support services on behalf of children in placement. This will ensure the provision of care designed to meet the normal developmental needs of the child within the larger context of the placement triad (1. Youth, natural family parents/guardians; 2. Agency staff and JPO; and 3. Placement Provider). This individual will have case responsibility which will include needed services to any other children remaining in the home.

Cases will involve cases shared between the Agency and Lycoming County Juvenile Probation Office (LJPO) and will be assigned according to the established Shared Case Responsibility protocols determined by the Agency and LJPO. Case management services will include involvement in the referral/placement process, service planning, advocacy for treatment services, visitation planning, documentation, permanency, and discharge planning. Case management services will emphasize addressing family of origin issues to help stabilize the home environment for families with delinquent youth that have been removed from the home. Permanency services will also include guidance for the youth and resource family directed to the resolution of an issue or issues related to the goals and objectives in the FSP/CPP.

The extent of caseworker involvement and frequency of contact with members of the placement triad will be determined by need and level of service being provided to the youth in care as well as any other children that remain in the home. At times, this will require flexibility in working after hours or weekends to provide the necessary crisis intervention and support to local resource families. This position will be supervised by the Ongoing Unit Services Supervisor.
