Job Description
  • Provides ongoing information, emotional support and counseling to residents and their families. Serves as the facility in-house resource in behavioral management leading the interdisciplinary team in developing environmental and therapeutic behavioral interventions, including assists nursing staff in maintaining a restraint-free environment.
  • Contacts residents and family member/responsible party within 48 hours of admission. Completes social history within 7 days of admission.
  • Provides information in conjunction with the Finance Department, regarding Medicare, Medicaid, and other financial assistance programs available to the resident. Issues the Last Covered Day Notice (NOMNC) when skilled coverage is being discontinued.
  • Completes all necessary state and federal forms in a timely manner this includes PASRR, MA 401, MA 408.
  • Develops a written plan of care for each resident that identifies the problems/needs of the resident and the goals. Reviews and revises care plans and assessments as necessary, at least quarterly. Develops and maintains a good rapport with all services involved with the care plan to ensure that a team effort is achieved in developing a comprehensive plan of care. Completes MDS sections as assigned. Documents interventions and meetings in the interdisciplinary notes.
  • Discusses discharge potential and plans at time of admission with resident, family and interdisciplinary team. Coordinates discharge planning when indicated and assists the resident and family to identify and locate any needed community resources. Works with the case manager to assist in arranging family meetings for Short stay residents, and is responsible for arranging the meetings for Long term residents.
  • Assist with resident council and discusses any issues or concerns with department leaders/ DON/ NHA. Assist with facilitating the Alzheimer's Support Group meetings held on the Springhill campus.

Schedule:

  • M-F 8:00am-4:30pm

Qualifications
  • Bachelors in Social Workor Masters in Social Work
  • Experience in medical/long term care setting preferred
