Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substances abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. The Psychiatrist is a key member of the Case Management Team in West Philadelphia. This position provides psychiatric assessment, evaluation and diagnosis of individuals in Outpatient and Assessment Programs and Substance Abuse Services.
Some of the duties include:
- To attend and participate in participant case conferences as co-treatment team leader: oversee counselors and managers in the formulation and monitoring of treatment plans.
- To clinically supervise the psychiatric treatment of all participants by monitoring and ensuring the provision of an acceptable level of clinical practice, in compliance with funding source requirements and Agency policies and procedures.
- Manage and coordinate medication administration and distribution for participants.
Analyze service delivery gaps; identify and recommend alternative service plans with Program Director(s) and/or management staff.
- Evaluation includes an initial psychiatric assessment and recommendation for specific services.
- Monitor MH Outpatient treatment plan documentation, charts and records, to ensure quality and reliability.
