Job Description/Essential Functions:

Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues and substance abuse. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strengths and choices.

Our Suburban Behavioral Health Services Region is seeking a dedicated Employment Psychiatric Rehabilitation Specialist to fill the following Full time opening at our Chester county program. The Employment Psychiatric Rehabilitation Specialist provides Psychiatric Rehabilitation Services (PSR) site based or in the community as assigned. Facilitates the development of employment and other community resources and opportunities. Ensures all clients receive the level of and/or have access to the services/supports they choose; also receive and maintain their participation in vocational or community based activities. Monitors and track client progress of assigned caseload; participate in the development of individualized rehabilitation plans.

This position receives supervision from the Program Director and is responsible for

* Coordinating employment development process for vocational opportunities and systems of coverage for the transitional employment program.
* Establishing and maintaining resource networks with employers and other vocational providers/opportunities.
* Providing and facilitating skill development and support which include daily living, socialization, educational/vocational supports and opportunities, and leisure/recreation activities planning.
* Establishing an ongoing dialogue and participate in case conferences on behalf of clients with other members of the rehabilitation team.
* Maintaining a close liaison with the local community and area service providers/agencies to facilitate program coordination and provide for constructive input and/or alternative resources to current psychiatric rehabilitation plans.

Job Qualifications:
A successful applicant will have the following:

* Bachelors degree or Associates degree and 1 year work experience in mental health direct service, or CPS certification and 1 year paid or volunteer experience, or a High School diploma/GED and 2 years work experience in Human Services, including 1 year of mental health direct service.
* Certified Psychiatric Rehabilitation Practitioner (CPRP) certification preferred.
* Experience with vocational or employment services.
* Must have excellent oral and written communication skills, as well as be detail-oriented and able to complete a high volume of documentation in a fast-paced environment.
* Strong computer skills are required, as well as Internet research abilities.
* Ability to work independently and as part of a team.
* Knowledge of mental health system and supports.
* Must possess a valid driver's license with an acceptable driving record.

Scope:


* Frequent talking and listening which includes interacting with participants who are at different stages of recovery.
* May be required to assist with crisis intervention in emergency situations including educating and supporting participants in the development and implementation of WRAP and Advanced Directives.
* Requires lifting and/or carrying and the ability to fold, kneel, stack, stoop and bend which may include direct client intervention.
* Must be available for local travel.
* May require crisis intervention for emergency situations.
* May be required to report to work during emergencies including inclement weather.
