EXPERIENCE REQUIREMENTS:Five years of clinical management experience. The ability to use computer software. Prefer familiarity with electronic medical records.
Minimum five years professional nursing experience, including two years supervisory level experience.
The Nurse Clinical Coordinator, in partnership and collaboration with other healthcare professionals, manages specific clinical and administrative functions related to the operations of the Family and Community Medicine clinical practice at the 833 Chestnut Suite 301 site under the supervision of the Nurse Manager, Medical Director, and Department Administrator. The RN Clinical Coordinator is responsible for organizing daily clinical workflow, working toward best practice principles/guidelines and participation in change management processes particularly those toward CPC+ functions: Access and continuity, Care Management, Comprehensiveness amp; Coordination, Patient amp; Caregiver Engagement, and Population Health.
As part of the nursing supervisor team, develops, coordinates, and supervises support staff performing clinical activities; serves as liaison between physicians, clinical, clerical and administrative staff and front desk.
