Position Summary

RHD New Start II is currently offering a sign on bonus of $4000 and a retention bonus of $4000


The Registered Nurse working for RHD'S New Start II program provides medical care to individuals involved in RHD's Mental Health and D&A detoxification services. The RN will also provide medical services including medication administration to all persons receiving services provided by RHD's New Start II.


RHD employees and programs in the City of Philadelphia are required to follow the COVID vaccine mandate. Revised PDPH guidance allows for new hires to be partially vaccinated at the time of hire. This means they have received at least one shot in a two-dose series or a single dose in a one-dose series before beginning in-person shifts. The final dose must be received within 8 weeks of hire (start date). This currently does not include boosters.


Essential Duties and Functions

  • Maintain nursing documentation and medication records.
  • Follow all agency, MCO, County and licensing regulations.
  • Supervise consumers to ensure safety and health of all residents.
  • Review quality assurance of medication monitoring.
  • Assessment of client health and emotional needs,
  • Follow agency protocols for assessment, documentation and reporting or client issues and status.
  • Provide nursing care such as wound care, diabetic counseling and etc. as needed
  • Communicate with other members of the nursing staff including report at change of shift and informing Nurse Manager, Director, Clinical Director and program team of importance matters.
  • Team with staff during crisis intervention to deescalate and re-direct client during periods of emotional distress.
  • Counsel and support clients regarding goals, feelings, and policies of program. Support staff in therapeutic process.
  • Coordinate, interaction with family members, ICM(Intensive Case Manager),adjunct staff, and other professionals as needed.
  • Complete assessments as required by written, verbal, or electronic protocols.
  • Complete required documentation and complete requisitions forms for testing
  • Assist with organization and ordering of medical supplies
  • Supervise client submission of urine for analysis, completed in office testing and /or complete requisitions for testing at an outside laboratory
  • Assistance with meal, baths and cleaning may be required
  • Attend trainings and staff meetings monthly
  • Provide training to the staff about medical conditions/considerations that pertain to the population served
  • Remain awake and alert on all shifts worked
  • Complete client supervision(location, medical condition)
  • Facilitate educational groups with staff and /or residents
  • Pour & Dispense medication to clients
  • Monitor the intake of medications
  • Document in Electronic Health Record(EHR)
  • Escorting client when applicable

Qualifications

  • A graduate of a school of professional Nursing (AD, hospital, or BSN); BSN preferred .Registered Nurse Available to work past shift hours if applicable. Required to work rotating holidays. Experience in a drug and alcohol detox setting preferred

RN License

Valid Driver's license

ACT33/34 clearance.FBI clearance,as needed

  • Medium: exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects.
