* A.A. degree in the social sciences or health care related field and one year of relevant experience in the MH/MR field or High school diploma and two years experience in the mental health field; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.
* Very good communication skills.
* Ability to work independently and as part of a focused team.
* Requires the ability to lift, carry, fold, kneel, stack, stoop and bend which may include direct intervention with residents.
* Possess manual dexterity and fine motor skills.
* Possess a valid driver's license.
. Ability to provide and implement services to residents, as identified in their individual program plans.
. Ability to participate in the analysis and identification of service delivery gaps, changes and problems, under the guidance of the Team Coordinator; implement appropriate action plans.
. Responsible for maintaining program reports and documentation logs; including medical logs, incident reports, healthcare related visits and activity/daily logs.
. Ability to coordinate the planning and design of leisure/recreation time activities.
* May require frequent crisis intervention in emergency situations.
* May be required to report to work during emergencies including inclement weather. |
