Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

Currently, we are seeking a Nurse Manager who takes pleasure in helping others live a fulfilling life.

This position will provided services in licensed community homes located in Delaware County, Pennsylvania with support of paraprofessionals. Multi-disciplinary services are community-based with emphasis on integrating individuals into their community. This position combines the care and skill of a nurse with the leadership skills and responsibilities of a manager. In addition to having the educational and clinical experience as a nurse, one should also be knowledgeable about financial management and practice policy.

Some of the duties are:

Provide care and supervision of all assigned residents to ensure a safe, healthy, secure and pleasant environment, which promotes and reinforces resident growth and development.

Coordinate and deliver patient care through the utilization of the nursing process including assessment, planning of care, implementation, resident and family education and evaluation of care.

Coordinate and monitor the overall daily operation of residential home, ensuring appropriate service provisions to residents.

Maintained a healthy, collaborative team environment.

Supervisor and train staff.

Monitor the financial needs and expenses of the program .

Job Qualifications:
A successful applicant will have the following:

Bachelor's (Master's preferred) degree In Nursing and a current State of PA license as a Registered Nurse

Minimum 2 years medical/surgical experience

Three to five years of nursing supervision skills with the ability to supervisor multiple locations

Minimum 2 years experience in Wound Care, G and J Tubes, Trach and Ventilator Care

Experience working with individuals with intellectual disabilities preferred

Strong communication skills both verbally and written

Able to work independently and with all levels of staff and interdisciplinary teams

Experience in teaching/guiding paraprofessionals in disease management and carrying out health-related procedures

Experience in use Microsoft Office Suite for record keeping, form development, and data base design and maintenance.

Possess valid driver's license with acceptable driving record

Able to travel throughout the community locally

Must be able to assist with crisis intervention in emergency situations

Scoop:
Requires sitting for long periods of time.
Light lifting and/or carrying, not to exceed 50 lbs.
Frequent talking and listening.
Manual dexterity.
