Job Summary: Under the direction of/in coordination with the Program Director, Clinical Coordinator, Case Manager, or OP Mental Health Professional: Completes psychosocial assessments/evaluations, actively develops and implements treatment plans, provides therapeutic interventions, and facilitates referrals both within the LVHN continuum and throughout the community, in order to address patients' on-going mental health needs. This position will have a direct reporting line to the Program Director, Psychiatry.

Minimum Requirements: Bachelors degree in Social Work, Psychology, Counseling, Nursing or related health/mental health field.

Preferred Qualifications: Two years social work experience in a health/mental health related field is preferred.
